|gdrive=The google doc link to pull from. A GDocs item must be shared and available to anyone that has the link. You need only the unique file name from the doc link, located between the /d/ and the /view (see the screenshot below for the highlighted section).
|author=The author (you). Use a username, link to your user page, put an email. Will take various wiki link formatting.
|othercred=Other credits (contributors, etc.)
